Photo Release Statement
We often take photos of your teeth for communication purposes with yourself, labs, and specialists that we work with. To protect your privacy, identifying information will NOT be included.
We ask that if you require a change to your reserved appointment time to please give us as much notice as possible, a minimum of 48 business hours is required to avoid a $75 cancellation fee.
